Approvers on distribution list missing when Workflow started.

in Design Tools & Output Technologies - JDE1 (MOSC)
I created a group distribution list wiith 4 approvers, each at a different level. When I initiated the Workflow, only the second and fourth approver were added with tasks.  The first and third do not appear in the task flow. 

Could something be missing in the setup for the to approvers omitted, or does someone know of issues in the release.  This is new development, so there is no history from a previous release of tools.

I am working with Tools Release 9.1, update 3 installed.
