New Menu Definition - HTML Tab not rendered

Hello All,

I've defined a new sub menu (menu type = HTML tab). The new menu has a single function included. I've added the new menu to an existing menu (Self Service Maintenenance) which is utilised by a self service/HTML responsibility. Unfortunately, the HTML tab will not render when the responsibility is opened. Any ideas? The cache has been cleared and a new IE session was initiated to no avail!

Please  see detail below - the menu in question is highlighted yellow.
