Urgent - Not able to attach a file to a PO

Hi,

There is an issue wherein user is not able to attach a file to a PO. Upon checking the status, it shows 'Requires REapproval'. Checked in the Action History and there is no pending items. The last status is Approved.

Can someone help me with this.
