Order line submitted for one org is getting created in another org after processed by process order

I created line 1 in an order for org A through a custom form and pass this line to process order api to create the order in oracle. Line 1 is created for org A in oracle.

Now I add another line 2 to this order through the custom form for a org B and pass this order to process order api. But the line in oracle is created with org A instaead of org B. This happenes randamly and does not happen always. Infact I am able not able to replicate the issue in test instance but we randamly get this issue in production instance. What could be the issue?
