Error when opening Create Incident Form

Hi

We've upgraded from rel 12.0.6 to 12.1.3 and getting errors ORA-01403: no data found, FRM-40735: CUSTOM1 trigger raised unhandled exception ORA-06502. This is after we've created a new folder.
