Deferred Status

When Requisitions/PO's approved by the user within the approval limit, some of the Requisitions/PO's are going to Deferred status.  As soon as Workflow Background Process is run with Deferred Status to Yes, these  Requisitions/PO's are Approved. We are following supervisor based hierarchy.
Can some one help why this is happening to some specific Requisitions/PO's ?
