Failure Set Not populating the Failure Code, Cause Code & Resolution Code.

Team,
How to Personalize the Self Service "Work Order Completion" form so when we click on the Failure Set link and eventually find the specific Combination of Failure, Cause & Resolution code. By clicking on that combination it should populate back all three codes.
