Responsibility for same name given two time in 11i

Dear all,We are facing the problem in our production server that some users are having the unique responsibility for two tmes if we end date it it can allow end date after save then it again show the responsibility without end date we are using the 11.5.10.2 application please see below attachment i am attching the screen shot also This can happend only for some users not all users
