In RTF template Check box issue

Hi All

We developed a form using BI Publisher version 10.1.3.4 .1 We desinged a rtf template ,in that we are using check box ,as per the oracle bi publisher guide we tried to give checkbox but still we are not getting exact check box but we are getting very small diamond box with a small dot instead of right mark .For that see the attached output pdf.

Please see the attached rtf ,output pdf and actual requirement.

Please reply me asap.
