VM fails to start

We have an issue starting a VM.
Starting the VM: Neither VM nor the VM's media is registered. 
When I try to add the VM, the message is
"Medium cannot be closed because it is still attached to 1 virtual machines." 
When I look in the Virtual Media Manager, the medium is present, and shows as being "Not attached" to amy VM.

How can the VM be attached and show as  Not attached in Virtual Media Manager?
