Embed a form on a microsite - remind me please

Hello
I'm trying to be a little more creative with my forms and how they look on a landing page/microsite.
You can see our basic one here
I want to be able to embed the form in a page where I have images and text as well as the form.
I recall I need some code, something like $M##### etc.
Can anyone help me with that please?
