Resource unavailability form and production downtime

Oracle EAM:

Please guide me where to mention resource down time information. Also guide me how to set up the resource unavailability form as mentioned in the document id 947243.1. I have set up the profile option as mentioned in that document but still the resource unavailability form is not available.
