Attention: To align with new corporate security policies, and to match policies adopted by other communities within Oracle, the My Oracle Support Community access requirements will change beginning September 18, 2025. I require HTML page of Service Request,Is there any menu to attach with field service reponsibility?

I need to use extensible attributes setup to meet specific requirements to use check box and radio bottons set/fields.
