Hot insertion of FC CARD in Exadata Cell server

hi,

i am receving Chassis:Action:Major Alarm regarding Host Insertion of all 4 FC CARD from ILOM of Exadata Cell servers .

its happening again and again after certain during like 14/15 days.

please take a look at the relavant log from ILOM(event log) and OS(/var/log/message)

Event from ILOM:
1325  Chassis  Action  major  Thu Jun 28 08:01:15 2012  Hot insertion of /SYS/MB/RISER1/PCIE1/F20CARD

Log from OS:
Jun 28 07:01:08 cel06 kernel: usb 1-3.2: USB disconnect, address 39
Jun 28 07:01:10 cel06 kernel: usb 1-3.1: USB disconnect, address 38
Jun 28 07:01:12 cel06 kernel: usb 5-1: USB disconnect, address 19
Jun 28 07:01:13 cel06 kernel: usb 5-1: new low speed USB device using uhci_hcd and address 20
