How do you migrate existing ASR manager server instance to ASR on Ops Center 12c?

So it seems that ASR is now bundled into Ops Center 12c. 
I have an existing ASR manager, and also a separate Ops Center 11g server which I plan to upgrade to 12c.  I would like to consolidate my two management servers (ASR and Ops Center) into just the single Ops Center server.  

The Ops Center 12c documentation is extremely high-level and just covers the very basics.  There is no mention of moving an existing ASR instance into an Ops Center 12c ASR. 

Has anyone done this?
