Debrief and Notes from another task show in current task

We are in the process of migrating our Oracle MFS PDA users to Oracle MFS Laptop. Several users have reported seeing debrief notes in a currently assigned task that are from another and older task. We also have cases where debrief lines from an older task are showing in a current task. In both cases the current task is created after the date shown in the debrief lines. This seems to indicate that sequence numbers may be overlapping from the PDA and Laptop but we don't see why this would be the case.

We are starting to get concerend that we may have to drop and recreate over 1,000 users. Any ideas why this is happening?
