Applying Patch 9239090 on Oracle Linux 5.8 64 bit server, upgrading R12.1.1 to R12.1.3

Dears,

I applied the patch 9239089 sucessfully as a pre requisit for applying 9239090.

the patch proceed for a while then i got the error attached in a document.

please help and advice.

This case is of sevirity 3 that cause "Severe Loss of service", kindly help and advice as soon as posible, i do appreciate a quick reply from any of you gents.  

i tried to run adpatch a second time and i got the errors attahed "9239090#2"
