Purchase Requisition status "Inprocess"

Hi Experts,

We are into 11i and using Employee - Supervisior Hierarchy

Requester raised Purchase requiistion and submitted . It went to next approver.

Approver approved the Requisition ,but still Requisition is "Inprocess" and requestoe got following notification

Approval workflow database error occurs while approving Purchase Requisition 231562

Contact your system administrator to identify the problem and to use SQL*Plus or Workflow Monitor to reactivate the approval workflow where it left off.

Give your system administrator the following information:

PL/SQL procedure: PO_APPROVAL_LIST_WF1S.UPDATE_ACTION_HISTORY_APPROVE
Error: ORA-01403: no data found
