System crashes with Forms server error when saving Debrief

Hello,
We have applied latest CS, CSF, CSP patches in our instance. Every time when i enter the Debrief line (material / Labor / Expense) in the SR form & save it, the system shuts down / crashes. Error attached. It was working good before. I will upload the patch list we promoted to the instance recently.
