Unable to add employee in R12.1.3...FRM-40735: ON-INSERT trigger raised unhandled exception ORA-0406

Hi Experts,

 I am unable to add an employee in HRMS.ours is a R12.1.3 Vision Demo. After entering and saving a record i am getting the below Error.

    ORA-01403: no data found

--------------------------------------------------------------
FRM-40735: ON-INSERT trigger raised unhandled exception ORA-04068.

Please help in solving this Issue.
