Deleting activities deleted dictionary values

When a user deleted 1600 activities from his project, activity code values from the EPS dictionary and the Global dictionary were deleted at the same time.  This led to these assigned values being deleted from other projects as well.  This user does not have access to add or delete values from the global dictionary directly.  This has happened at least once each in two of our Oracle databases. Has this happened to anyone else?
