Logo Not Showing on Bill Presentment Invoice

We have several customized Bill Presentment Invoices that we have been using for a few years now.  We are adding some new business and need to create an additional invoice.  I duplicated an existing invoice, moved the new logo (gif format) to the OA_MEDIA directory, added it as a new content item and added it to the new template.  When I generate the Online Preview, the logo displays as expected.  However, when I generate the Print Preview, or the customer generates the invoice through the application, the logo does not display.  What have I missed?  Thanks in advance.
