OTL: Timecard Workflow Restarted

A timecard was approved, and the user received the e-mail confirmation of the apprvoal.  15 minutes later, the timecard was in her supervior's queue again. Her supervior has to approve again. There was no rejections and resubmission of her timecard.

Went to workflow monitor and found two workflow for the timecard. How did the workfow restarted for the already approved timecard?

Did anyone experienced this before?
