Asset hierarchy problem - asset references itself as parent in mtl_object_genealogy

I am noticing an issue where occasionally an asset references itself as the parent asset in mtl_object_genealogy.  This causes an infinite loop hierarchy when you display the hierarchy using Maintenance Super User>>Assets>>Hierarchy.  eAM prevents users from intentionally creating a hierachy where an asset references itself as the parent and, so far, I have not been able to determine what is causing this to happen.  Has anyone else ever seen this or have any ideas?  I logged an SR and have gotten a successful data fix, but the issue continues to pop up and I have gotten nowhere determining root cause.  Any help
