The patch 4125550 installation ends with error: Error loading seed data for FND_RESPONSIBILITY:  APPLICATION_SHORT_NAME = BIS,
RESP_KEY = BIS_BV_SETUP,  ORA-00001: unique constraint (APPLSYS.FND_RESPONSIBILI
TY_U1) violated
ORA-06512: at "APPS.FND_RESPONSIBILITY_PKG", line 33
ORA-06512: at "APPS.FND_RESPONSIBILITY_PKG", line 525
ORA-01403: no data found

Followed this note's solution, but after the worker restart the problem still reproduces.
