Post Database Migration from HP-UX (Itanium) to RHEL 6 Linux (x64) : ALL EBS Patches fail with ORA-0

Hi Sureshkumar,Welcome to the new EBS Patching community.

This thread will address the following issue

Followed the ML# 1311487.1, Migrated my RAC (3-node) Database from HP-UX (Itanium) to RHEL 6 Linux (x64);
Source and Target RDBMS version is 11.2.0.3 ;
Oracle Application version : R12.1.1

Now, when I try to apply a patch it throws the below mentioned error :
Error while inserting into AD_APPLIED_PATCHES.

ORA-00001: unique constraint (APPLSYS.AD_APPLIED_PATCHES_U1) violated
adphistRecordInDBDataModelDone(): Error in calling adphistCreateAppliedPatch()

AutoPatch error:
adphistRecordDataModelDone(): Error calling adphistRecordInDBDataModelDone()


AutoPatch error:
Error calling adphistRecordDataModelDone()
