How do you store attachments?

We're currently storing attachments in the database. They are in the FND_LOBS table and apps_ts_media tablespace. The tablespace increase 4g weekly. Are you storing attachments in the databse? How you manage the growth? Or are you storing attachement outside of the database? How does that work?
