Cat code value no longer shows in header of FASTR when page breaks after upgrade.

Problem:  The state, which is a category code is no longer showing in the header of a FASTR when a page break occurs based on a change in catetory code, after upgrading from 7.3 to 9.2.

Solution:  The print sequence in heading flag, in the override defaults is blank.  The field must be populated in order for the cat code value to print. Populate the print sequence in heading field with a Y.
