F0709 missing supplier

We have had an instance where we have a supplier in the EE DBA instructions, however, when the payroll is run it creates an error.  When looking closer at it we discover that the F0709 does not contain the supplier.  One thing to note is that the supplier is in the EE DBA instructions by way of a custom update report that pulls the supplier from Benefits Administration.

Wondering if anyone else has experienced this before?
