JDB3400009 - Unable to Insert Records

   We have a UBE that when the user submits it, gets the error JDB3400009.   No records are written to the table but if the user goes and submits it a second time, the records get written and everything is fine.   Has anyone else had a problem like this and if yes,  did you figure out how to fix it?  

Log File looks like this when it doesn't work.
