Help with "apply CRS patch" failure during deployment of GI PSU?

in Patch Reviews - ENM (MOSC) Question
Patch 13906113 and 13906147 from note 427577.1.  I applied both patches and still getting failure during "apply CRS patch" with this error.  Files are there and accessible.  RAC, EM12c, Linux x86-64, 11.2.0.2.
