Error when trying to use Online Tax Form function in SSHR in R11i

Hi,
I am trying to use the Online tax Form function in Employee Self Service. When I log in as an employee and try to navigate, I get an error saying -

oracle.apps.fnd.framework.OAException: java.io.FileNotFoundException: /u01/app/hrd/comn/html/fwk/t/onlinew4_3527827.pdf (No such file or directory)
    at oracle.apps.fnd.framework.OAException.wrapperException(OAException.java:891)
    at oracle.apps.pay.selfservice.w4.webui.ReviewCO.processRequest(ReviewCO.java:263

The full error message and the screenshots are given in the attachment. The pdf file number is different each time I try. I tried for different users.
