Multiple workers failing related to HR API user hooks during 11.5.10.2 to 12.1.1 upgrade

 Multiple workers have failed while running the 12.1.1 upgrade driver. I have skipped the failed workers and continued with the patch as per doc  1413917.1 but more similar errors continue to pop up. So far more than 80 workers have failed and there is a good chance more workers will fail as I skip and continue to apply upgrade. What is causing this issue and how can I avoid these errors? Should I continue to skip the workers and continue with the patch regardless of the failure count?
