When attempting to apply patch 12972273:R12.PO.B,the following error occurs.

ORA-01658: unable to create INITIAL extent for segment in tablespace APPS_TS_TX_IDX

occurred while executing the SQL statement:

CREATE UNIQUE INDEX fnd_install_processes_u1 on
 fnd_install_processes(worker_id)  TABLESPACE  APPS_TS_TX_IDX
 storage(initial 4K next 4K)
