Workflow configuration not generating mail.(Urgent)

Hi,

we want to send a mail to customer on there query from CRM OD.

we have configured a workflow for customer request as per the attached document but unable to send mail.

whenever we create a customer request against any of the contact, we want to send mail for the acknoledgement to that contact.

Please go through the attachment and guide for further assistance or any more setup required other than  this.
