Header-Level Sales Supplement duplicating when Printing Quote to PDF File.

We have a production issue that just started to occur where Header-Level Sales Supplement is duplicating when Printing Quote to PDF File.  We only have 1 Sales Supplement at the Header-level, and no Line-Level Sales Supplements.  As the System Administrator, I am not aware of any changes to our Oracle EBS environment.  The issue is occurring with existing (not new) items on the quote.

Can anyone help me on troubleshooting this issue?
