Patch: 7461070 : R12.AD.B.1 : FAILED: file jai_cond_ren_col.sql on worker 3

Issue:

While applying patch 7461070 : R12.AD.B.1 getting following error:

There are now 133320 jobs remaining (current phase=A3):
   0 running, 0 ready to run and 133320 waiting.


ATTENTION: All workers either have failed or are waiting:

  FAILED: file jai_cond_ren_col.sql on worker  3.
  FAILED: file jaiinstchktabcr.sql on worker 15.

ATTENTION: Please fix the above failed worker(s) so the manager can continue.

Cause:
Start time for file is: Thu Jun 07 2012 09:44:20
