Patch 3612492:R12.GL.B - CONSOLIDATED PATCH FOR 12.1.3 fails generating forms

Hi,
We are upgraded our 11.5.10.2 instance to R12.1.3 and as part of post upgrade process applied 13612492:R12.GL.B - CONSOLIDATED PATCH FOR 12.1.3 patch.   The patch is unable to generate  GLXSTCAL.fmx with following error

Results of form generation:
(from file /caqa/apps/caqa/apps/apps_st/appl/au/12.0.0/forms/US/GLXSTCAL.err
AD Worker has deleted the original .err file)
