Patch 13111289:Linux x86-64:12.1.3:fails to compile the form BOMFDBOM.fmx

Hi,
during the application of patch 13111289.form BOMFDBOM.fmx failed to generate due to following errors.

The following Oracle Forms objects did not generate successfully:

bom   forms/US    BOMFDBOM.fmx


Lang patches are applied sucessfully.

invalids before/after : 280/280

Error in worker log file.
FRM-30431: Warning: Duplicate column name EFFECTIVITY_DATE in block MTL_ITEM_REVISIONS.
FRM-30047: Cannot resolve item reference COPY_BILL.UNIT_NUMBER.
LOV SERIAL_NUMBER_LOV
Form: BOMFDBOM
FRM-30085: Unable to adjust form for output.
