Unable to Provision Database Using Existing Installation

Hi Again!

This is my very first try to use database provisioning feature of enterprise manager grid control 11g. I followed section 5.4 Provisioning Oracle Database Using Existing Installation" of Administrator's Guide for Software and Server Provisioning and Patching 11g Release 1 (11.1.0.1.0) E16599-05.

I followed all the guidelines but my job failed and the same error lines are reproduced here i.e. Deployments->Database Provisioning Procedures->Prodedure Completion Status from here I click on the link under "Run" and from Status page click on the job name and on "Job Run: DIR_PREREQ_NAME_C84FC5AB4F05A9A6E040007F01005422" status is:
