how to attch file on Oracle Requisition after document submitted

Hi All,

Anyone know how to attch file on Oracle Requisition after document submitted.
our issue is, requester submited Requisition to approval, but forgot to attach file for more information.

the Approval ask by requset information and back to requester, then notification send back to requester. 
the Requester can open and attach file as per approval requested and submit the answer question.

the problem is, the approval still not see the attachment file


could somebody can help us..
