difference between patch set, upgrade

HI,

I want to know what exactly makes us to call which is patch set and which is called upgrade.

 Ex:
Assume: i have a database 10.1.0.2, if i want to bring it to 10.2.0.4 what is it called (upgrde? or patch set?)
what exactly differentiate between these 2.
finally.. what is patch set and what is upgrade (please explain me excluding CPU and PSU patch set)
