Need a sample Label format - rtf template

Could you please point me to a sample rtf template to be used for Label printing ?Or direct me on what Layout should I be using ?

I tried different layouts but could not achieve what I want.

Sample output

xxxx  xxxxx  xxxx
xxxx  xxxxx  xxxx
xxxx  xxxxx  xxxx
xxxx  xxxxx  xxxx

Just a regular label like format.
