How to prevent multiple "Spouse" type of contacts during same time period?

Hi,

We are on R12.1 Oracle HRMS.  We've had a few occurrences of the issue.  When a new hire starts, his contacts got entered into Oracle HRMS twice - once done by the employee via ESS, once done by a HR admin person not knowing that the employee did it. In our recent case, the employee entered his spouse via ESS, an admin person entered the same spouse via PUI form. They system took both records.  However this created issues and onfusion for the new hire enrollment process.  In the end, the employee's spouse shows up twice on the 834 file to BCBS - even
