SIM - Duplicate adjustments

I am seeing duplicate adjustments created in rk_inv_adjustment and RMS.tran_data tables for items during a stock counts. SIM is 13.2.3. Unable to recreate this scenario, but field users after performing stock counts can see duplicate adjustments. Anyone encountered this issue?
