IRC - when uploading resume from external site, system returns to upload page, but nothing is upload

We recently upgrated to R12.1.3 and our external site is not allowing candidates to upload a resume.  There is no error associated with this, it just simply stays on the upload screen, even after you select a resume to upload.  We use Resume Mirror to parse resumes, and have confirmed that they are receiving the resume when we upload it, but it "disappears" once it comes back into Oracle.  Has anyone experienced this with R12? We did not see this issue in our TEST instances prior to go live, so assume it is a configuration issue somewhere.  Any points in
