Forms Personalization - unable to create new record group after patches applied

Is anyone aware of any issues of why a new record group is unable to be created after the following patches were applied?

Rollback the conflicting RDBMS patches 12683462 and 13388061.RDBMS patch 13923804 --> RDBMS to 11.2.0.2.7RDBMS 12683462 patch for 11.2.0.2.7RDBMS 9877980 patch for 11.2.0.2.7Patch 14009893Patch 13979372Patch 9935935
We've had personalizations on the debrief form for years to restrict the LOV for the service activity and now all of a sudden it isn't working.  I've tried to recreate the record group, but nothing has worked.  All of the service forms were also recompiled and that didn't work either.  Any insights would be appreciated as we are currently unable to invoice.Thanks.
