Patch 6678700: ORA-20001 error while applying patch 6678700

Hi Team,

Customer is upgrading from 11.5.10 to 12.1.1
Patch 6678700 fails with following error

--From  u_merged.log --
ATTENTION: All workers either have failed or are waiting:

           FAILED: file invitmpl_INV.ldt on worker 16.

--From adwork016.log--
Time when worker started job: Thu Sep 13 2012 15:58:58

Loading data using  FNDLOAD function.
FNDLOAD APPS/***** 0 Y UPLOAD @INV:patch/115/import/invitmpl.lct @INV:patch/115/import/US/invitmpl_INV.ldt -

Connecting to APPS......Connected successfully.

Calling FNDLOAD function.

Returned from FNDLOAD function.

Log file: /oraap/tstr/appl_r12/apps/apps_st/appl/admin/TSTR/log/US_invitmpl_INV_ldt.log
Error calling FNDLOAD function.
