Applying patch 14020534 adrelink failed on AFPASSWD

We're applying patch 14020534 to our test E-Business application and got error code 8 during adrelink on AFPASSWD. 

We did check the permissions in the bin directory and that was fine.  Space is also fine on the server.  We're not sure what to do next or how to fix this. 

We told adpatch to go ahead in applying the patch since this is our test environment.  But want to make sure we don't have this error once we're ready to apply to production.

adrelink.log file shows:

Relinking module 'AFPASSWD' in product fnd ...

ld: 0711-317 ERROR: Undefined symbol: .iifgcg
ld: 0711-317 ERROR: Undefined symbol: .iifatr
