pop up questionnaire during open enrollment

Is there way to provide a pop window or a questionnaire that an employee can fill during open enrollment. I know we can make use of EIT's to capture the information from employees in self service, however most of the clients want this questionnaire to pop up on the benefits enrollment function itself, so that employee doesn't need to navigate to another section.

The question can be anything - but must be used for reporting through oracle. - so some solution that can store the answers against the employee record.

say like 1. is your spouse under different insurance ?

             2. do you have another insurance ?
